DESCRIPTION:Join us Thursday\, November 18 as we participate in The Miami Foundation’s Give Miami Day POWER HOUR!\n9:00 AM – 9:59 AM ONLY\n			\n				\n				\n				\n				\n				Throughout the pandemic\, HSCMD has worked tirelessly to ensure ongoing\, uninterrupted services throughout our diverse community. Our dedicated teams of case workers and therapists are connecting with mom and new families using consistent outreach and to provide education\, address physical needs\, and offer psychosocial counseling. \nPregnancy and the early weeks of parenthood – typically times of celebration\, family joy\, and connection with loved ones – continues to be drastically affected by the COVID-19 pandemic. These are frightening times as the COVID-19 crisis continues to change our way of life. \nThe Healthy Start Coalition of Miami-Dade (HSCMD) works to improve the lives of pregnant women\, children\, and their families by providing a connection to free care coordination and customized services based on each family’s goals and needs. We focus on lessening the dangers associated with pre-term birth\, low birth weight\, infant distress\, and poor growth-related outcomes. \nPregnant women and families are seeking our support now\, more than ever before\, as they embark on their parenting journey during this unprecedented emotional and economic crisis. \nWe embrace our mission with passion and professionalism\, uplifted by the strength and love our mothers and their entire families share with their young children. \n\n\n \nReasons to Give\n\n\n\n\nWhat are Prizes?\n\n\n\n\nPrizes help your selected organizations earn extra dollars. These hourly and milestone opportunities range from $500 to $1\,500 in extra dollars for the winning nonprofits. An organization is only eligible to win one prize. Please note that the winner of the Early Bird Prize will be eligible to receive an additional prize during Give Miami Day. The full list of prizes will be available in November.\n\n \n\n\n\nWhat are Champion Pages?\n\n\n\n\nChampion Pages are an easy way to support your favorite nonprofit on Give Miami Day. You can create a unique URL to share with your network\, set fundraising goals\, keep track of gifts made on the day\, upload photos and videos as well as a personal story about why you’re supporting a specific nonprofit.\n\n \n\n\n\nWhat is the Bonus Pool?\n\n\n\n\nAll gifts between $25 and $10\,000 will be eligible for a bonus gift from the Bonus Pool. The bonus gift percentage will be calculated by dividing the total Bonus Pool dollars by the total eligible gifts. For example\, in 2019\, the bonus gift percentage was approximately 5%. DESCRIPTION:The Healthy Start Coalition of Miami-Dade (HSCMD) is a community-based non-profit organization designated and funded by the State of Florida Department of Health and the Agency for Health Care Administration to serve as Miami-Dade County’s prenatal and infant care coalition. HSCMD partners with local community-based organizations and healthcare professionals to plan\, coordinate and provide high quality health and education services to women of childbearing age\, children to age three and their families.  \nOur Mission is to reduce poor birth outcomes\, premature births\, and improve health and developmental milestones by providing education and linkage to resources in Miami-Dade County. Please help us to ensure that pregnant women\, infants\, and their families are linked with the services they need to ensure the best possible outcomes. Please mark your calendars and donate on Give Miami Day by clicking here on November 21st \n  \nWho We Serve: \nHSCMD is the primary organization in Miami-Dade County for high risk pregnant women\, babies and women between pregnancies – regardless of marital\, economic or immigration status. We help clients address their pre-natal and post pregnancy needs. Post-partum women and children up to age three are also eligible for services. \nWhat We Do: \nHSCMD oversees multiple programs in Miami-Dade County\, including the Connect\nProgram\, the Moving Beyond Depression Program and the Fetal Infant \nMortality Review (FIMR) Project. Additional outreach programs are also available to serve our \ncommunity. \n• Connect Program: Through universal screenings\, Connect identifies needs and links families to the programs that best serve them. This coordinated intake and referral process ensures the best fit for families and streamlines access to services. At the same time\, it improves program enrollment and retention rates\, reduces duplication of efforts and builds a broader early childhood system of care in Florida. Connect helps pregnant women\, caregivers and families with young children by providing a one-stop entry point for needed services\, such as education and support in childbirth\, newborn care\, parenting\, child development\, food and nutrition\, mental health and financial self-sufficiency.  \n• Moving Beyond Depression (MBD): Moving Beyond Depression (MBD) is an evidence-based\, comprehensive approach to identifying and treating depression in mothers participating in home visiting programs. MBD seeks to foster rapid recovery and promote positive outcomes for mothers and children. MBD uses licensed mental health clinicians to deliver In-Home Cognitive Behavioral Therapy (IH-CBT)\, a treatment developed for the home visiting setting that consists of 15 weekly sessions with a one-month follow-up booster session.  \n• Fetal Infant Mortality (FIMR): The FIMR Project of Miami-Dade County works to better understand the issues associated with fetal and infant mortality and to develop strategies that improve local perinatal systems of care. Through reviews of local fetal and infant deaths\, FIMR identifies strengths\, areas for improvement and development of policies in service systems in order to safeguard infants and families.  \n• Healthy Start: The Healthy Start program provides universal risk screening to all of Florida’s pregnant women and infants to identify those at-risk for poor birth\, health\, and developmental outcomes. At-risk women and infants are eligible for Healthy Start services\, including care coordination\, psychosocial counseling\, and education on various maternal and child health issues in a home-base or setting of their choice. \n  \n• Outreach efforts include: \n\nHome Based Care Coordination \nPrenatal Education\nScreenings: Perinatal Depression\, Smoking Use\, Substance Use\, Intimate Partner Violence\, Child Development \nStress Management Education – Mothers and Babies\nInter-conception Education\nFamily Support Plan \nIndividualized Plan of Care\n\n\nHSCMD offers enhanced services when there is a lack of resources in the community and funding is available. Those services often include: \n\nBreastfeeding Education \nChildbirth Education \n Nutrition Counseling \nPsychosocial Counseling\nTobacco Counseling \n\n\n  \nThank you for your support!
